In late October 2003, food handlers working at Chi Chi’s Restaurant at the Beaver County Mall in Center Township, Pennsylvania failed to thoroughly wash the green onions when preparing salsa. This led to biggest hepatitis outbreak in American history. 660 people became ill and four died. Individual and class action lawsuits forced Chi Chi’s out of business.
