Eight-year old Andrew Sheldrick was a fun loving, caring, healthy young boy living in Ontario, Canada. One night his mother gave him what she thought was his normal prescribed sleep medication. Andrew never woke up after taking a powerful muscle relaxant – three times the amount that would be toxic for an adult. The Sheldricks filed a multi-million dollar wrongful death claim against Floradale, citing the cause of death was a lack of training for Floradale technicians. The error was the result of a medicine mix-up at the pharmacy and Andrew’s mother didn’t recognize it because the medications looked alike.

Andrew’s mother hopes that in the future pharmacies will be required to report all errors and this will result in mandatory training.
