Mexican chain fails to train food prep workers basic sanitation procedures

In late October 2003, food handlers working in the kitchen at Chi Chi’s Restaurant at the Beaver County Mall in Center Township, Pennsylvania failed to wash the green onions before preparing salsa, which the restaurant provided to every table for free. This resulted in the biggest hepatitis outbreak in American history. More than 660 people became ill and four died, including two high school students. Individual and class action lawsuits forced Chi Chi’s out of business.
